Position Description:Promote the vision of the local Healthy Start Initiative. Provides case management services to recipients of the Health Start Program. Serves as lead coordinator and care manager of services to clients as part of an interdisciplinary home visiting team that includes outreach/health advocates. Participates in various in-house, professional, and community based committees and boards. Attends workshops, seminars, and in-house training. Maintains high level of proficiency and mandated contact hours. Maintains documentation and reports according to agency and program policies.
Minimum Requirements: Must be Registered Nurse, with a minimum of two years experience in maternal and child health and high-risk pregnancy. Ability to work in an urban setting with a diverse population of varying levels of socioeconomic status. Case management and home visit experience desired. Leadership experience in community coalitions, maternal/child and perinatal health experience specifically related to infant mortality prevention. Knowledge and experience with coordinating the fetal infant mortality review committee, ability to analyze data from statistics and other sources and make appropriate recommendations to address problems identified. All degree nurses must be in possession of a current Michigan license to practice nursing in Michigan. Ability to drive private automobile, have a valid Michigan Driver's License, and good driving record.
